Payroll Deduction Form

Payroll Deductions Forms must be received by September 1st.  Payroll Deductions will automatically renew each year until you notify PENC that you wish to cancel. Click here to access the form.  You may email, fax, or mail the form to us, but we must have your signature on the form.

Payroll Deductions are withdrawn from your payroll checks in the months of October to May each year.

  • Educator PRD is $24.75
  • 1st Year Teacher PRD is $11.25
  • Classified/Support Staff is $11.25
